EPS Form Maker ver 1.0 is a free web-based tool that can create a fully working customized form for you. You can then install this form onto your website and start collecting data from your website visitors. The frontend display of the form created is highly customizable because it depends on your template. In other words, the form could be easily integrated with your current website design. You can specify to store the data collected into your mysql database or redirect the data to your email address or both. The form also comes with a backend panel that let you review the data that you've collected and/or deleted those data that you no longer needed. You only need to know a bit html (and html only) in order to use this system with ease.
